One conversation. One document. Three lasting benefits.

The Personal Health Timeline™ doesn't just help your doctors — it transforms how you engage with your own health and what you leave behind for your family.

🩺
Built for Your Doctors
Today's physicians juggle multiple platforms, fragmented records, and disconnected systems just to piece together a patient's history. Your Personal Health Timeline™ gives every provider — from your primary care doctor to a new specialist — an accurate, chronological view of your health from the moment they open it. No more screen-shuffling. No more starting from scratch.
💡
Empowering for You
When you walk into an appointment with your own organized history, the conversation changes. You're no longer trying to recall dates and details under pressure — you're an informed, prepared participant in your own care. The timeline gives you confidence, clarity, and a stronger voice in every medical interaction. And often, the process of telling your story is clarifying in itself — including events that were meaningful to you but never made it into your chart. Because your timeline is built from your story, not just your records, it can capture what the system missed. And sometimes, that's exactly what a provider needs.
👨‍👩‍👧
Invaluable for Your Family
Your health history is your family's history too. Genetic patterns, hereditary conditions, and treatment outcomes are information future generations deserve to have. Whether you're coordinating care for an aging parent or leaving a legacy document alongside your estate planning, this timeline is a gift that carries lasting meaning.

Still searching for answers?
Your story may hold the pattern no chart ever captured.

Medical charts record what providers documented. They don't always record what you experienced — especially when symptoms were dismissed, episodes went undiagnosed, or patterns only became visible years later. The story-first process is designed to surface exactly these moments. What felt significant to you belongs in your record.

Your health history exists.
Is it organized?

Most people's medical histories are scattered across clinics, partial records, and memories they've never had reason to organize. The Personal Health Timeline™ was built for the people who feel that gap most.

🔄
Complex & Ongoing Health Journeys
When you're managing a chronic condition — or still searching for answers to symptoms that have never been fully explained — your health history isn't background information. It's the foundation of every conversation with every provider you see. The Personal Health Timeline™ was built for people navigating exactly this: the appointments, the specialists, the gaps between diagnoses. It captures not just what was confirmed, but what was significant to you — including symptoms you sought care for but that were dismissed, episodes that never led to a diagnosis, and patterns no single provider ever saw in full. Because the timeline is built from your story, it can hold what your chart may not. And that fuller picture may be exactly what finally leads to answers.
🌿
Retirees & Pre-Retirees
You've accumulated a lifetime of health experiences — and as care needs grow, having a complete, organized record becomes essential. Whether you're transitioning to Medicare, adding specialists, or simply want to walk into every appointment prepared, the Personal Health Timeline™ ensures your providers always have the full picture from day one.
🤝
Caregivers & Family Health Advocates
Whether you're an adult child coordinating care for an aging parent, or a parent navigating the healthcare system for a child with complex or ongoing medical needs — you carry someone else's health story alongside your own. The Personal Health Timeline™ gives every family health advocate an organized, accurate record they can bring to any provider, share in any emergency, and rely on without having to reconstruct details under pressure.
📜
Legacy & Estate Planners
Alongside your will, power of attorney, and financial documents sits one more record your family will be grateful you created. A documented health history captures diagnoses, hereditary patterns, and treatment outcomes that future generations deserve to know. It's one of the most meaningful — and most overlooked — documents you can leave behind.

One document.
Your complete story.

Your Personal Health Timeline™ is a structured, chronological record built to work in clinical settings. It's built from your health story — told in conversation, captured in detail, and shaped into a document that speaks for you in every clinical setting you'll ever enter. Physicians today navigate a maze of portals, platforms, and fragmented records to understand a single patient's background. This document cuts through all of that — giving every provider an accurate, complete view of your history at a glance.

  • Major diagnoses & chronic conditions
  • Surgeries, procedures & hospitalizations
  • Medication history over time
  • Symptom onset & progression
  • Undiagnosed events & dismissed symptoms
  • Family medical history
  • Pregnancies, births & significant injuries
  • Notable test results

Timeline Update

Your health doesn't stand still — and your timeline shouldn't either. Recommended every 6 to 12 months, or sooner if a major health event occurs, this service keeps your record current as your health evolves.

Every client receives a personal tracking template to log medical visits, diagnoses, medications, and health events between sessions. Joy reviews your updates and incorporates them into your timeline — so your record is always accurate and ready to share.

Wondering what the process looks like?

From a 15-minute consultation to a story-first guided session — every step is unhurried, personal, and centered entirely on you.

Your consultation is free.
Your history is priceless.

Step one costs nothing. Schedule your complimentary consultation and discover how The Personal Health Timeline™ can work for you.

Your health history isn't a form.
It's a story — and stories reveal more.

Most health records are built from checkboxes, intake forms, and fragments of memory recalled under pressure. The Personal Health Timeline™ is built differently — through conversation, guided by someone trained to listen.

The story you tell is more complete than the data you recall.

When someone asks you to provide your health history, you produce a list. When someone asks you to tell your health story, you remember things you forgot you knew — connections between events, patterns across decades, details that surface only when the right question is asked at the right moment.

Joy brings health coaching skills to every session — the art of asking open-ended, memory-evoking questions that create space for your story to emerge naturally. This isn't an interview. It's a guided discovery process that moves chronologically through your life, using conversation techniques to surface health events, relationships between diagnoses, and context that a checkbox never could.

The result is a timeline that reflects not just what happened, but what it meant — making it more useful to every provider who reads it, and more meaningful to you and your family.

"Most people know far more about their health history than they realize. The right conversation brings it forward."

— Joy Duginske, Founder

Stories surface patterns — a recurring symptom, a condition that connects to a family history, a medication that preceded a diagnosis.

Chronological structure jogs memory — moving decade by decade through a life unlocks events that a direct question never would.

Health coaching engagement creates trust — clients share more when they feel heard, not processed.

Narrative context makes the document richer — the Notes column in your timeline captures what forms never ask for.

And it captures what your chart may not. Symptoms that were dismissed. Episodes you sought care for but received no diagnosis. Patterns that only become visible when your full story is laid out in one place. If it felt significant to you, it belongs in your record — and this process is designed to bring it forward.

That's the story-first approach at work.

A health advocate who believes your story matters to your care.

Joy Duginske — Founder, Record Your Health History
Joy Duginske
Founder · Record Your Health History

Every experience I’ve had has led me to this gap.

I’m Joy Duginske — a health and wellness advocate focused on helping individuals take a more informed, active role in their care.

My career spans health insurance, social services, medical clinic communications, and private health coaching, giving me a broad, systems-level perspective on healthcare. Across each role, my work has centered on empowering people to better understand and manage their health.

My perspective is also personal. After navigating a complex diagnosis myself, I came to see knowledge and communication as the foundation of effective healthcare. Having a Personal Health Timeline™ can fundamentally change how healthcare is delivered.

My graduate training in Communication, along with research experience in health communication, deepened my understanding of how clarity — or the lack of it — shapes the patient experience. Time and again, I’ve seen the same challenge: patients and providers often lack access to a clear, complete health story at the moment it matters most.

I created Record Your Health History to close that gap. By organizing and preserving essential health information, I help ensure that patients, families, and providers can make more informed decisions with confidence. My goal is simple: to bring clarity, continuity, and empowerment to the healthcare experience.

Frequently Asked
Questions

Everything you need to know about The Personal Health Timeline™ — how it works, what it includes, and whether it's right for you.

How long does a Record Your Health History session take? +

Session length varies by individual — and that's by design. Most sessions run between one and three hours, depending on the complexity and length of a person's health history.

Someone with a relatively straightforward history and a shorter timeline may complete their session in about an hour. A person with a longer life history, multiple chronic conditions, or a complex medical background may need closer to two or three hours to do their story justice.

The session is unhurried. There's no rush to get through it quickly — the goal is accuracy and completeness, not speed. Many clients find the process surprisingly meaningful, as it's often the first time they've told their full health story from beginning to present in one sitting.

Can't I just create my own Personal Health Timeline™? +

Yes — you absolutely can. And the honest answer is: have you? Or will you?

Most people know they should have an organized health record. Very few ever create one. It keeps getting pushed down the list, because it feels overwhelming, time-consuming, and hard to know where to start — especially when the information is scattered across memory, old records, and years of different providers.

That's where this service changes everything. Joy guides you through your history in a structured, supportive session, handles all of the organization and formatting, and delivers a professional document you can actually use. No staring at a blank spreadsheet. No trying to remember what year that surgery was. No endless delay.

When it's done, you, your doctors, and your loved ones will be grateful you checked this off your list — once and for all.

How long is The Personal Health Timeline™? Will every health event in my life be included? +

The Personal Health Timeline™ is a robust, structured document — but it is intentionally focused on the health events and facts that matter most, not a word-for-word account of every medical encounter you've ever had.

Think of it this way: a routine annual physical with no notable findings doesn't need its own entry. But a diagnosis, a new medication, a surgical procedure, a significant symptom pattern, or a specialist referral absolutely does. The timeline is designed to capture what is clinically meaningful and chronologically significant — the events and conditions that a physician needs to understand your health history clearly and quickly.

It will not — and simply cannot — include every lab draw, every routine visit, or every minor illness. What it will include is the kind of organized, substantial health record that most people have never had. For most clients, that document is more comprehensive than anything they've ever seen in one place. And it will be immediately useful every time they interact with the healthcare system.

Should I bring my medical records to the session? +

You're absolutely welcome to — and if you have records handy, they can be a helpful reference. But most people don't have a neatly organized file of their health history sitting in a drawer, and that's completely fine. You don't need one to have a meaningful, complete session.

What you do have is your story. The session is built around your memory and your experience — guided by a structured, decade-by-decade conversation designed to surface the health events that matter. Most people are surprised by how much they remember once someone helps them walk through it thoughtfully. A medication list, a surgery date you look up on your phone, a note from your doctor's portal — any of these can add detail and precision, and you can refer to whatever records you have as you go.

The goal isn't to replicate your medical chart. It's to capture your health story as fully and accurately as possible — using every resource available to you, starting with the one you already carry: your own knowledge of what you've been through.

Is my health information kept confidential? +

Absolutely. Confidentiality is foundational to this service. Your health history is among the most personal information you can share, and it is treated with the utmost care and discretion at every stage of the process.

Session recordings are used solely for Joy's reference while building your document and are deleted once your final timeline is delivered. Your files are stored securely and are never shared with any third party for any reason. The only person who receives your completed document is you.

If you have specific questions about data handling or privacy, Joy is happy to discuss them during your free consultation.

What happens after my timeline is delivered — can it be updated? +

Yes — and keeping it current is one of the most valuable things you can do. Your health doesn't stand still, and neither should your record.

Joy offers an Annual Timeline Update service, recommended every 6 to 12 months. Every client receives a personal Health Update Tracking Template — a simple form to log new diagnoses, medications, procedures, specialist visits, and other health events as they happen throughout the year. When you're ready for an update, you return the completed form to Joy, who reviews it and incorporates the new information into your timeline.

The result is a living document that stays accurate and useful — not a snapshot that becomes outdated the moment it's delivered.
